In my opinion, the absolute best, most effective and easiest to configure (for most people, anyway) spyware protection is your brain.

Never click the OK button on a popup window.
Always close popup windows with the X or by right clicking its icon on the Taskbar, left clicking Close, never use the Cancel button.
Never install software or open a file unless it's downloaded from a trusted source
If you're not sure if it's a trusted source, use a search engine and do some research on it before accepting their "upgrade" or "install our software to use our site"
Never install, run, open or otherwise use any software or file obtained through a file sharing program unless you know the person you're downloading from
Don't visit porn sites
Don't visit warez sites

Other than that, Spyware Blaster seems to be as good as any. Windows Defender works well, too.

Excellent responses. To add:

Never click on "You've won a free...whatever", "Click here for your free Horoscope", or the worst: "Your computer is infected with spyware, click here to download a free spyware remover". Also stay away from screensavers, pretty pointers, or smilies.

Windows Defender will not run on Me. Spybot and Adaware were ok about three years ago, but there are much better alternatives now. IMO, the best AntiSpyware program is SUPERAntiSpyware (http://www.superantispyware.com/). No program will get all the spyware on an infected computer, so every once in a while run a scan with BitDefenders online scanner.
