Port Forwarding/Static IP Setup - DNS Server Question!

I'm on portforward.com at the moment attempting to forward my ports for a program. At present all I have filled in is the following:





Now the next thing I have to do is (according to portforward.com) "Enter the dns servers we prevoiusly found into the two DNS Server boxes".

Well when I type "Ipconfig /all" into Command Prompt it gives me THREE DNS servers, not TWO as shown below.





So which of these three do I input into the Preferred/Alternate DNS Server boxes?

As you can probably tell, I'm really confused as to what to do.
