patch panel probs

Hi peeps,
does anyone know where i can get a simple wiring diagram for the back of a 16 port patch panel, showing how to share
1. a telephone feed
2. a broadband line
I only have to split the each "line" into 5 outputs but i may as well do the whole panel for future needs.
