troublesome modem

Can anyone help me?? I have a dial up connection, and it takes me anywhere from ten minutes to two hours to make a connection.
I contacted the manufacturer of my modem to help me. They say I need
the following information in order for them to assist me.
Chip id, audio/modem add in board, and driver version. I tried to find them on my comp, but was unsuccessful. Thanks in advance.
