Over the past few months I have been having problems with my start bar icons changing. They don't disappear they just look odd. I took a couple of screenshots to show what the icons look like

Occasionally my icons will go completely black and the number 6 will appear on my internet explorer right next to the "Back" tab. I can usually fix the icons by using Tweak UI but the fix isn't permanent.

Hi Julibras, this is not a malware/spyware issue, your log is fine. Try this, run a search for ShellIconCache and when you find it, delete it and reboot. If you still have problems after doing this, rightclick on your Desktop, choose Properties > Effects and select Default Icons.

Some 3D games that I play cause the same thing to happen when they do not end and terminate themselves correctly, sometimes my resolution isn't even what it is supposed to be.

Anyway, what I had to do for it was go into EFFECTS (like AnnMarie suggested) and then take the check off of "Show icons with all possible colors", then hit APPLY at the bottom, then re-checked it, then hit APPPLY again, then OK.
